How do I schedule an appointment?

How do I make an initial appointment for a workers compensation claim?
The patient needs to contact their employer or case manager, and the employer or case manager will call to make the initial appointment with Dr. Boes.
If I have already been treated by another orthopaedic practice, what do I need to do to make an appointment with Dr. Boes?
Please call (919) 344-0862 and the appointment specialists will determine whether you will need to provide physician’s notes, surgery notes or x-rays for review prior to the appointment.
What should I bring to my appointment?

In order for us to better serve you, it is very important that you bring the following items to your visit:
- Photo identification
- Your health insurance card and co-pay
- A list of current medications
- Any and all previous x-ray, MRI, CT, or bone scans
- Your referring physician and primary care physician’s name
- A list of your questions and concerns
- Patients under the age of 18 must be accompanied by a parent or legal guardian
- Completed patient forms as appropriate
What are your hours of operation?
We are open Monday through Thursday from 8:00 AM – 6:00 PM and Friday from 9:00 AM- 5:00 PM.

What insurances do you accept?

It is best to check with your insurance company before making an appointment to determine if your visit will be covered.
Is a co-pay required for follow-up visits?
Your insurance policy determines whether or not you have a co-pay for your follow-up visits; however, under most insurance policies, a co-pay is due at each visit.
What is the best way to request medication?
The most efficient way for you to help us take care of your medication refill requests is to complete the online prescription refill request, or have your pharmacy call us or fax your medication requests to (919) 863-6904. Once the medication request is processed, your medication will be called or faxed into your pharmacy.
In some instances, we may require that patient be seen in our office prior to medication renewal. Any request received after 12:00 noon, will be processed the following business day.
How do I get my short-term disability or Family Medical Leave of Absence forms completed?
These forms are usually provided by your employer or insurance company. In effort to streamline the process of completing the large volume of forms we receive daily, we are unable to complete short-term and long-term carrier-specific forms. Instead, we will attach to your form a computer-generated form, which will provide all the information that your insurance carrier is requesting. It may take up to 5-7 business days to process your form. There are several ways you can get these forms to the administrative assistant for your doctor or mid-level provider:
